Job Summary: Our VISTA project will engage the community to develop resources for a new housing development that will provide support and services for persons below the poverty level. The VISTA will also help organize current homeless residents into a Resident Advisory Committee to do outreach in the community.

Job Summary: The Outreach Counselor will provide services and support, as needed, to individuals who have moved out from PLASE’s Transitional Housing Facility and into Permanent Housing. The purposes of these services would be to maintain an individual in their residential setting, increase their functioning and daily living skills and try to prevent further homelessness on their part.

Nurse
Job Summary: Assesses and tracks client health needs in an effort to provide intervention; performs other job related duties as assigned; with an emphasis on persons with AIDS/HIV+. Provides services for PLASE residents living in our housing facilities. Provides health assessment, education, and monitoring of their condition re: HIV; mental illness, chemical dependencies, diabetes and other serious health conditions. It is expected that if the nurse is made full-time that approximately 120 clients will be served during the year.
