Outreach Counselor

The Outreach Counselor provides services and support to individuals who have moved from the Transitional Housing Facility of Project PLASE to Permanent Housing. He/she will make frequent visits to the homes, and various living situations of PLASE residents in order to assess their progress.

The Outreach Counselor develops and accesses community-based contacts and services for individuals. He/she trains and actively helps the individual in daily living skills to include: personal hygiene, housekeeping, and budgeting as well as social and interpersonal functioning skills. He/she addresses areas around chemical use recovery, mental health, medical, emotional needs, education and employment.

SSVF Case Manager

The Housing and Hope for Veterans Case Manager will provide intensive case management services and support to Veterans and their families who may have complex needs such as serious and persistent mental illness, history of substance abuse, history of incarceration, history of chronic homelessness or history of underemployment. He/she will assist Veteran families with accessing the community resources necessary to meet their individualized permanent housing needs including, but not limited to, mental health, medical, financial, educational, vocational, residential and social support services. He/she monitors and documents consumer progress and outcomes.
